Remote Monitoring

Traditionally, biometrics like blood glucose, blood pressure, and weight have been shared with your healthcare providers via a handwritten log.

Today, it is easy and safe to share these biometrics with your healthcare team so that they can help you stay on top of managing your chronic conditions.

Virtual Classes

In this class, our experienced team of providers will help you and a group of like-minded classmates learn how to make lifestyle changes that support healthy habits.

You’ll learn from and connect with others, stay engaged, learn about mind and body health, develop healthy habits, and sustain biometric improvements.

How often does the class meet?

  • 1.5-hour sessions once a week for 6 weeks

Emotional Wellbeing

The health of our mind has a profound impact on the health of our body. Our Behavioral Health Specialists will help you identify any barriers to your emotional well-being.

The ‘Ekahi Wellness team will teach you ways you can better manage stress, establish healthy habits, and safely increase your physical activity.

Nutrition

Our registered dietitians will help you understand the impact your diet has on your diabetes. They’ll partner with you to develop healthier menu options so you can live well with chronic conditions.

  • Get help understanding how the food we eat influences our health.
  • Learn about a plant-based diet (recommended).

Health Coaching

Our case managers are your personal guide to health and wellness. They’ll help you build a care plan to reach your goals.

This care plan will include joining a hui and developing the optimal schedule for you to meet one-on-one with our providers.

Medication Reviews

Our clinical pharmacists review your medications and supplements with you. They’ll help you learn what each medication or supplement does and how to safely take them.
