How do I get started?

Getting started is easy! Call us at (808) 777-4000 and one of our support specialists will be happy to assist you. Our staff work closely with your physician to obtain a referral as well as your previous medical records.

Will I still see my regular physician?

Yes. Your regular physician will remain in charge of your care. Our goal is to empower you to achieve success in your health. We will work closely with you and your physician to ensure that your health goals are met.

Is the program a covered benefit of my insurance?

The ‘Ekahi Wellness Program is a covered benefit of most insurance plans in Hawaii and we will work with you to determine eligibility. 

How will this program affect my medication?

The ‘Ekahi Wellness Program is not a substitute for regular medication, but we do help participants better understand the side-effects of their medication, as well as how lifestyle changes can help them decrease their medications in some cases.

Will I need to see all the providers each time I visit?

Our Wellness Navigators will connect you with the appropriate providers, as they all play an important role in maintaining your health.

We now accept most health insurance plans. Please fax us a completed referral form so we can determine plan eligibility.

Is my patient eligible?

We now accept HMSA commercial plans. To determine if your patient is eligible for the ‘Ekahi Wellness Program, please call us at (808) 777-4000 or fax a completed referral form to (808) 447-0571.

What can I expect?

We will communicate with you any recommended care plan changes regarding the issues that your patient was referred for.

How long will the wellness team be involved?

The length of engagement will depend on the goals and needs of your patient. When goals are met, we will work on a mutually agreed upon maintenance plan.
